چکیده

Consider the following scenario: Every day before work, you stop at a neighborhood coffee shop and order a cup of coffee with cream and no sugar. This scenario repeats itself for years without incident. Then one day, after you drive away with your coffee, you notice that the workers forgot the cream and put in sugar instead. The next day you mention the mistake, and the coffee shop manager apologizes, so you pick up the coffee as usual. Two weeks later the same mistake happens: sugar, no cream. You are mildly annoyed and express your dissatisfaction, so the manager apologizes again and refunds your money. All is well for a

Error assessment in man-machine systems using the CREAM method and human-in-the-loop fault tree analysis

Background and Objectives: Despite contribution to catastrophic accidents, human errors have been generally ignored in the design of human-machine (HM) systems and the determination of the level of automation (LOA). This paper aims to develop a method to estimate the level of automation in the early stage of the design phase considering both human and machine performance. Methods: A quantita...

Impact of Human Error Modeling on Failure Rate and Optimum Routine Test Interval of Protection System

Power systems should have acceptable reliability in order to operate properly. Highly available and dependable protective relays help to obtain the desirable reliability. The relays should be periodically evaluated during specific intervals to achieve the mentioned characteristics. The Routine Test Interval (RTI) should be optimized in order to economically maximize the reliability of the prote...

Mitigating Single Event Upsets

The Soft Error Rate (SER) is expressed as Failure-in-Time (FIT) units, defined as one soft error occurrence every billion hours of operation.Often SEUmitigation is not required because of the low chance of occurrence. However, for highly complex systems, such as with multiple high-density components, error rate may be a significant system design factor. If your system includes multiple FPGAS an...

Trellis Coded Mapping for Peak to Average Power Reduction in Sfbc Mimo Ofdm Systems

Space frequency block code orthogonal frequency division multiplexing (SFBC-OFDM) systems has inherit drawbacks of high peak-to average power ratio (PAPR) signals, and constellation shaping with trellis is a promising approach for reducing both peak power and bit error rate of SFBC-OFDM signals. In practice, reliability will improve in SFBC-OFDM by employing coded modulation. In this work, it h...

Evaluation of Human Reliability by Standardized Plant Analysis Risk HRA (SPAR-H) method in the Dialysis Process in Ebne Sina Hospital, Shiraz

Background and Objectives: Human errors in dialysis care can cause injury and death. One of the basic steps to increase reliability in this critical process is to analyze the error and identify the weaknesses of doing this process. Methods: The present study is a descriptive-analytic cross-sectional study. The SPAR-H method was used to identify and evaluate the probability of human error in th...
